Camp Staff Report Forms

To obtain an Incident Report Template click here. Click “Use Template,” fill out completely online, print, have all parties sign, scan, and email to finance@passportcamps.org and give the original to your Assistant Director to include in the Money Pack. The Incident Report is used to document behavioral concerns, rule violations, conflicts, or unusual situations that occur during camp which do not involve physical injury. This form helps leadership track concerns, address issues appropriately, and ensure a safe and positive camp environment. Examples include camper behavior issues, disagreements between campers or staff, or situations that are concerning but do not result in someone being physically hurt.

To obtain an Accident & Injury Report click here. This form must be completed anytime a camper or staff member experiences a physical injury or accident, no matter how minor it may seem. This includes situations requiring first aid, ice packs, nurse care, or outside medical treatment, as well as small scrapes, falls, bumps, or more serious injuries. The Accident & Injury Report is used to document what happened, how it occurred, and what care was provided to ensure proper follow-up and recordkeeping. This form should be completed as soon as possible after the injury occurs.

Anyone using their personal vehicle for camp purposes should complete a Camp Mileage Form by clicking here. Note: you must click this link to create and submit a new mileage form each week.
