GLP HIGHLIGHT: SMARTWAIVER FORMS
*Medical Release Forms never seem important until they do.*
You may have noticed a change in the way Passport handles medical release forms. In an effort to streamline the process & make the experience as effortless as possible for all parties, Passport has moved Medical and Photo Release Forms to SmartWaiver, a secure online platform.
Last week our registration team emailed you a unique SmartWaiver URL specific to your group. If you are registered for more than one program, you should have received a unique URL for each program your church is attending. New things can be scary, but we think even our most physical-form-loving Group Leader will come to love SmartWaiver.

How does it work?

How will I know which of my parents have completed the form?
When a parent/guardian completes the form, SmartWaiver will email the guardian & the group leader (as indicated on your church’s registration) copies of the forms. When you receive the email, we strongly encourage you to download the form right away, as the download link & corresponding file expire in 3 days. This downloaded copy is for your own records, as the Summer Camp Teams will not need additional copies.
How should I keep up with who has/not completed the forms?
When you are emailed a copy of your student’s form, the subject line will include the student’s name. We suggest creating a folder within your email Inbox and moving all SmartWaiver confirmation emails to that folder. As camp approaches, you can see at a glance the students you have forms for and, by the matter of deduction, the ones you don’t.
Oh no! These forms print huge!
When printing forms, they are 4 pages long, but the text is very large. We recommend printing 4-to-a-page, since these are still legible and use much less paper. Our tree friends appreciate your heart for creation care.
